Pre-registration

Part I: Pre-admission center

As part of pre-registration, you will need to visit the pre-admission center. The pre-admission center is located on the first floor of Medcenter One, 300 N. Seventh St., Bismarck and is open 8 a.m.–5 p.m. No appointments are necessary.

When you visit the business office before your surgery, you will need the following information for insurance and billing purposes:

• Your insurance company card, name, address and policy number—if you do not have insurance, please call 701.323.8589
• Your social security number
• Other information such as copies of your power of attorney and living will—power of attorney and living will information should be done prior to pre-registration

If you are unable to stop by the pre-admission center to pre-register, please call 701.323.6542 or 800.218.2032 as soon as possible before surgery. Cost estimates and/or payment options can be made available. If you have further questions, please call 701.323.8589.

Part II: Nurse interview

After your pre-admission center paperwork is complete, a nurse will interview you to go over your medical history and answer any questions you have.

This interview can be completed between 8:30 a.m. and 5 p.m. Mondays through Thursdays, and between 8 a.m. and 4 p.m. Fridays. If you are unable to come during these hours, please set up a time to meet with a nurse by calling 701.323.6894 or 800.247.1873. Nurse interviews take place next to the pre-admission center on the first floor of Medcenter One, 300 N. Seventh St., Bismarck, N.D.

Please bring a list of your medications, including doses and how often you take them. During the interview, the nurse will tell you what to expect before, during and after your surgery. You will be given instructions for after your surgery to review at home. You will be asked to sign consent forms.

We encourage you to ask questions and to let us know about any special needs you have. We want your visit to be as comfortable as possible.
